Social Work Examiners: Forms and Applications
On this page you will find forms related to licensing. These forms can be downloaded and uploaded as needed with applications to the Board.
Please note that prior paper applications for both initial licensure and renewals have been removed from this page and are no longer accepted.
Beginning January 13, 2022, all applications will be completed electronically and may be accessed through our new online licensing portal. Current licensees, and individuals who recently held an active license, should have received a registration email with a unique link to login and access their records. Individuals who have not held an active license since before 2017, or those who are applying for a license for the first time, must create a new account with our online licensing portal to access and complete online applications.
