Chiropractic Board: Background Report Instructions
Background reports from the National Practitioners Databank Report (NPDB) and the Federation of Chiropractic Licensing Board’s (FCLB) Chiropractic Information Network/Board Action Databank (CIN-BAD) are required for initial licensure.
To request a report from the NPDB, follow the link, register, and request a Self-Query: https://www.npdb.hrsa.gov/. Additional instructions can be found on the NPDB Website: https://www.npdb.hrsa.gov/pract/selfQueryBasics.jsp.
To request a report from the FCLB/CIN-BAD, follow the link to register and request a disciplinary report: https://memberbenefits.fclb.org/account/login.aspx?ReturnUrl=https://fclb.org/cin-bad-query-form.php. Please note, if you do not have an account with the FCLB/CIN-BAD, you will need to contact the organization via phone or email to have an account created for you.
